I hope this email finds you well.

My name is Dr Rajni Toppo, and I’d like to take a moment to introduce myself and my organization, Creative Directions.

I’d like to share information about our programs at Creative Directions, Inc., with the goal of collaborating and building a referral partnership to better serve our communities.
We are a CARF-accredited Behavioral Health organization that has been serving North Carolina communities for over 20 years. We accept all major insurances, including Medicaid, and provide transportation for participants as needed. You can learn more about us at www.creativedirectionsinc.org.

Our Programs and Services
  1. Psychosocial Rehabilitation (PSR) – Helps individuals manage stress, loss, and mental illness (including PTSD and depression). This program is ideal for those rebuilding their lives, such as ex-offenders and veterans. Program hours are Monday–Friday, 9 AM–5 PM, and include community integration activities and field trips designed to promote confidence, coping skills, and social connections.
  2. Peer Support Services – Provides personalized guidance and assistance with daily life management. Participants typically receive 4–6 hours of support per week, focusing on organization, goal-setting, and stability.
  3. Supported Employment – Teaches both hard and soft skills to help members obtain and retain meaningful employment. Designed for individuals who have faced challenges maintaining steady work.
  4. SAIOP (Substance Abuse Intensive Outpatient Program) – Supports individuals striving for recovery from substance use. Sessions run three hours per day, three days per week (Monday, Wednesday, and Thursday), offered both in-person and via telehealth. Day sessions are 10 AM–1 PM, and evening sessions 6 PM–9 PM. A new cycle begins February 9th, led by Dr. John Howard at Greensboro, NC. The typical program length is about 12 weeks.
  5. Therapy Services – Provides individualized support to help clients process life challenges and move forward positively. Offered in person and virtually at our Graham, NC location.

Re-entry Housing  &  Behavioral Health Home Care Services
Each home includes security cameras, a phone, and an alarm system. Residents stay for 3 to 9 months, during which we assist with employment, community integration, and transition planning for permanent housing. Residents must be in by 8 PM nightly (alarm is set 8 PM–8 AM), and transportation is provided.
We also maintain other housing and AFL (Alternative Family Living) arrangements within Alamance and Guilford Counties. When possible, we develop additional housing in new areas based on community need.

Creative Directions is credentialed through Vaya, Trillium, and Alliance MCOs, making us well-positioned to support a wide range of member needs. Our mission is simple: to help individuals build stability, achieve independence, and experience lasting success.
