Hello everyone,

Are any of you using Ubited We Rides's "Building an Individual
Transportation Plan" booklet (12 pages) for your own use, travel
traininers or promoting it for agency use? It looks a little long, but
didn't know what your experiences may be with it?

Hi Amber,

We created a survey of a similar nature recently. 

Doing this we found that, to get participation, it had to be relatively short, mostly multiple choice (as little writing as possible) and administered one on one whenever possible.  Does that help or were you looking for specific experience with this tool?

The trip planning booklet was a starting point for us. I think Cynthia Kloppel can provide details how she creates individual trip plans.

Hello Amber, 

This booklet & the Easter Seals Travel  Training has formed the basis of my work in creating individual trip plans.  Overall the booklet & training provided me with an awareness of the level of detail needed per each the trip planning for each & every individual person in need of help. 

I have supported individual callers in the 3 plus years of service with the creation of individual trip plans ranging from plans created to help DSS TA clients get to a Job Club, individuals searching for means to travel out of county for training or work, & individuals searching for means to travel out of county for medical appointments including one for a surgery.  Actually this one particular person called for help with a trip plan because she had not been able to keep her scheduled surgery appointment due to lack of transportation & needed to find those strategies/trip plan prior to rescheduling.  Note that with medical trips, particularly to hospitals, that a call to the Social Workers will be very helpful in finding out what patient supports are available within the hospital or connected to the hospital such as reduced price overnight lodging. Each trip plan begins with a conversation of major points such as trip origin & destination, date & time of travel,  trip purpose which may also help you in terms of finding funding or knowing that there is not funding available, are they receiving Medicaid or Temporary Assistance- and could these services be of help with the cost of the trip?, do they have money to take the trip or are they in need of funding, when funding is needed - do they meet eligibility for Medicaid if  this is a medical trip or if other, what funding is available?, plus individual health concerns including & not limited to ambulatory, cognitive, language barriers & any other concern that will impact on travel.  

I have not promoted the booklet for other agency use. However as we work on building a local database of transportation options & examples&strategies for individual trip planning, we will remember to load this trip plan template into the system!  

I am enclosing 1 file for a medical trip plan & a file which was recently created for a simpler trip.
