Reimbursement Request Form

As an act of pro action, I created a Reimbursement Request Form. We are not ready at this time to go to usage but we should be soon.

Some rules:

Fill out completely
Receipts are necessary.
It is a .pdf file so electronic submission is OK. Attach your receipts as a picture.

Snail mail is acceptable. Send printed form with receipts attached. Mail to:

Moravia, NY 13118

Large expenses should be approved by the core committee in advance before submitting a reimbursement request.

Reimbursement turnaround time shall be based on committee approval and available funds.
