incorrect data is populated in form ranger

to NV Form Ranger Add-on
incorrect data is populated in form ranger
* it was working yesterday
* but today, the data is incorrect - i cant see the pattern
* populated data is somewhat a merged from reference sheet,
* there are no duplicates
anybody experience this?

Is the source spreadsheet a Google Spreadsheet? Not an Excel or other brand of spreadsheet that Google can read?
What type of question are you updating?
