Categories not showing on form

Although the Range appears to be populating correctly in the UI, even after refresh and closing the sheets to re-open, the information is not populating.

Any thoughts?

Refreshing the screen on the form is required to see the most current version.

Can you create a spreadsheet and form that is failing and share with glua.joe @ gmail . Com
