Form Ranger not working

Thank you for providing your user key, the error that is populating for this key is indicating that there are duplicate values for a  drop-down list question you are using Form Ranger with. I am seeing this same message for many users who submitted their key and am wondering if this is a Google Sheets or Google service-related issue or if you do have duplicate values for the same question?

formRanger only repopulates the form correctly when I open the add-on and click Start. Any help would be greatly appreciated. I am wondering if the issue is this spreadsheet was a copy from a previous form. 

I think I am going to try to create a new options tab. I will let you know how I make out.

I am seeing a problem for about a week across 3 separate forms.  Sometimes (6 times within the last week) one of the drop-downs shows a #REF.  When you go into edit mode and open Form Ranger it freshens and displays the drop-down values.  Again across 3 separate forms.  Also, the lists/drop-downs that are being affected vary.

I would be looking at the spreadsheet containing the updated data.  How complicated are the calculations?

The spreadsheet is not #REF.  Once a enter the Form Ranger App in the Form, it updates the Drop-down values.  The data isn't changing, just not updating in the app.

My formulas are not very difficult.  Just an Arrayformula nad sort.  One had the unique formula to remove duplicates.

Did you review the information in red at the top of the group description?  I suspect you have duplicates.

I do not have duplicates and yet the form is not updating once a submission is made. The sheet seems to be doing what it's supposed to, but it is not communicating with the form.
