Incomplete question list

Hi! 

Today Form Ranger started omitting more than half my form's questions from the Question List, making it essentially useless. I had imported several questions, and moved sections around. When I made a copy of the form, the problem persisted.

Is this likely a problem with just this form, or should I remove and reinstall Form Ranger?
