questions not populating

Hello, 

I've used Form Ranger successfully in previous years however when I revisited it today, it is not populating the question that I am trying to set up in my google form.  I believe I am following the correct steps, 

1.  Create a MC question in the form

2.  Open Form Ranger and start

3.  Select the response spreadsheet

4.  Select Sheet 1 where my choices are with a header.

5.  Populate question.

I was hoping to use it for the elimination feature as well.  If this isn't working, does anyone know of alternatives?

Update:  I have solved the populating issue.  It was due to duplicate date.

However, it now will not eliminate.

Check formula.  Are you setting the value to "" (null) or " " (space)?  If two rows have a space, that is a duplicate.

There must be at least one item on the list.  If all the items have reached the quota, then a new row must be created that indicates no items available.
