Cannot complete set-up on formRanger

to NV Form Ranger Add-on
After installing the add-on it prompts to complete set-up which will open a new tab/window. However when clicking the link complete set-up, the window that opens is an ERROR 404 - not found. The app is installed but there is no way to set-up nor use it. Is there an update or is this add-on no longer supported?

If I ignore the link, Formranger is apparently installed. But it doesn't appear in the list of the add-ons I have. Therefore I can't use it because there is no way to open it. I also had to google it to find it and install it. It does not appear in the list of add-ons when I open that window.

FormRanger should be available at the Add-on store when on a Google Form.
If you click on the "help", it will bring you to this screen. The Learn more icon doesn't work.
