Form Ranger not populating answer choices for only one question

If the solution for this exist, I apologize in advance for not being able to see how to correct my issue.

I have a Google Form, where all questions will populate answer choices except for one question using FormRanger. I am not sure why the one question will not populate. Especially because I used the same form last year and it worked. I have deleted the question and added it back several times. I am not sure what to do.

I need permission to see the form.  It would help if you told us which column was not loading.  Column A, B or C etc.

Most of the time I have found a duplicate entry in the list.

My permissions are set that anyone with this link can edit for both the form and the spreadsheet. 

On the spreadsheet I need Column G on the Rising 10th FormRanger, Rising 11th FormRanger, and Rising 12th FormRanger will not populate to their coresponding forms. 
I have only shared the Rising 12th Google Form.

I copied a sheet to look for the duplicate.
I added a column and did a unique.  
Different number of rows.  
Added a column and did a count to find out which value was duplicated.
Sorted the count to bring the duplicate to the top.

There was a duplicate as I suspected.

So what does this mean? Sorry I am trying to teach myself google stuff. In order for the FormRanger to work there can only be one of each entry. If a an entry is listed twice, cause it can fall under two categories, it will not work? How do I correct this problem? As I will need to do this on three different forms. Is it easiest to only list the entry once??? Sorry for my confusion.

It means that you have "(8178) Human Growth & Dev." in the list twice.  Remove one of them from each sheet and I suspect FormRanger will be happy.

I'm not sure why you have it in the list twice.  When the student makes a selection, you would not know which one they selected.

If there is a difference between the classes, change one of them.

You have a sheet for each question.  If the same list applies to multiple questions in the form, you only need one copy of the list. (one sheet)
