Does Form Ranger Only Work for Forms with One Question?

I'm trying to have a Google From populate several DIFFERENT questions from several DIFFERENT columns in the same spreadsheet. But it seems that Form Ranger isn't allowing for different questions in the From to populate from different columns in the spreadsheet. Is that correct or am I doing something wrong?

I think it should work on more than one question.
