Form Ranger not populating

I have set up a spreadsheet with my data in it but it will not populate into the question in the Google form. I have another question in the same google form from a different sheet that works just fine. The preview shows the proper items.
Why will the question not populate?

Never mind figured it out. Had some duplicates.
