Form Ranger "additional step" not working

to NV Form Ranger Add-on
When I install the extension, it successfully installs but requires me to click on a link for an additional step required to complete set up. The link takes me to a Error 404 page.

Any help would be greatly appreciated!

It should work without clicking on the link.

I need a little or maybe a lot more information.  

Where does it fail?  

Have you read the information about duplicate entries in the description of the group?

Do you have any duplicate values in the data?  See the not in the description of the group.

The same for me : the adon was working yesterday and not workinf today. 
@Joe Schmidt, when you say "no duplicate entries in the table", witch table do you meen? The on that we pick in?

The most common problem is duplicates int the list.  See the comment in the description of the group..

I have the same issue. I am using the formula to remove duplications, and it is still not updating. (There were no duplicates in the list to begin with)
