Auto populate of Dropdown List not happening

to NV Form Ranger Add-on
Good morning,
I have added Form Ranger to my form and followed the directions to get it to auto populate a dropdown list from a sheet but the list is not generating on the actual form.  Is there some common issue that I might be missing as this is the first time I've ever used form ranger?

You cannot have any duplicates in the list.  The list can't be greater than 999 items.
