Cannot set email template name and move beyond the screen

Hello,

I'm hoping someone can help me.  I have a spreadsheet that did not come from a google form, but I am using Formmule to send out emails.  I've done this many times before, but right now I am stuck on the "Set # of email templates and send conditions" screen.  I use Joe's error tab and I don't have any duplicate headers.  I have tried setting it up with 2 different accounts.  Any ideas?  

Joe, I can share the SS with you if you can let me know your email.

Unfortunately I can't uninstall the extension because it's installed to my OU.

For reference, Joe helped me.  I had a URL in my question as well as double and single quotes.  I removed the quotes and moved the URLs to the question description.  Things started working after that.

In the past, I have used identical form description and did not have an issue so this is something that has changed recently.

Single quotes should work.  Double quotes will not work.

I modified find formmule errors to identify links.

I think these errors started back in February.
