Send Conditions Malfunctioning // Not Sending

Hello,  I set up this formmule months ago and I needed to make some edits to the triggered emails.  The send conditions (NULL, NOT NULL) are no longer present and the selections for send condition based on the columns (over 30, A-AR) of the spreadsheet also do not populate.   See screenshot.  I have logged in from 2 different accounts and the same result.  

Also, all merge tags are gone when I have toggled thru the set up tab via add-ons on the top tool bar of google sheets.

What are my next steps to get this form and formmule up and running?

It looks like Form Mule is set to merge on the wrong sheet ( it is not on the sheet with all the data)

Make sure you are only logged into one Google Account and select 'Open' from the Form Mule menu, then close out the module and go back to the drop-down menu and select 'Choose source data and set merge type' ( image below is the page you want to be on) Reset it to the correct sheet, then your conditions and merge tags should be visible.
