Form Mule Not Loading

to NV Form Mule Add-on

Form mule is not loading and I can't figure out why... Can anyone help? I found a duplicate tab and deleted it, but I still cannot get it to load.

I also had the same error and it never reloaded after doing multiple things (i.e. double checking for duplicate templates, enabling and disabling the app, making sure no one else was within the app, etc). We ended up having to duplicate our spreadsheet and reattaching it to our google form. I would also like to follow this and how to keep this error from being generated.
