issue with creating templates

Why cannot I create more than one template and why are  not all the columns available for me use for the Condition and why don 't have a the button to save the templates?

Hi.  

Check to make sure you have headers on every column and that none of the columns have duplicated header names.  For example, you cannot have two columns with 'CONTACT' as the header.  They need to be unique.   If you have a blank column, you have to have something in the header cell otherwise, form mule won't include the following colums as part of your data set. 

This is the most common cause of issues.
