Form Mule Trigger Not Working

The form mule trigger is not running on form submission. I have toggled it off on the main screen, reset the triggers, cleared my cache, and rebuilt the job twice.

You have tried the things I would suggest.

Is the FormMule set to run on the response sheet?

You may need to create a new output spreadsheet from the Google Form.
