Bugs on Send Conditions Page

Hello, I have a number of Google Sheets (all linked to different Google Forms) that use FormMule to seamlessly send alert emails, and generate Form Edit URLs and case numbers.  However, sometimes when I seek to set up a new sheet (which I am trying to do now), I find the Send Conditions Tab is missing key functions: the send conditions box (where I would type "NOT NULL"), the Template names, the back button, the next button (see attached pic).  I am consistently getting this bug now.  Is there something I can do to stop this?

Look at the description of this group.  You likely have a problem with the headings.
