Emails sending for some Templates but not all, & Send Status not updating

I created a new form on May 2, 2019. I created 2 templates to send on Form Submit. One template sends, but the other does not. The Send Condition for the template that works is 'Send for all rows'. The Send Condition for the template that does not send is when the field value = Yes.

Also,  the Send Status fields did not update for the template that worked.
