Unable to enter # of emails/templates

I have a form/spreadsheet that has been sending notifications through formMule just fine for a year. The notifications stopped working at least a week ago, and when I went in to fix it, it just kept spinning and wouldn't open the formMule window. Eventually it sent me an email:

"It appears the tab with ID undefined in the Google Sheet [link] cannot be found. It's quite possible someone deleted this tab after setting up formMule. To stop this error, we've removed the time trigger from this formMule instance. To fix this, try re-running the formMule setup steps and choose a different tab to merge from."

However, there was no change the spreadsheet and the tab still exists. (Also, I only used a form trigger, not a time trigger.)

Now it lets me go into the slide the Form Trigger to on, but when it goes to the Set # of Emails step, my previous templates are blanked out, and it keeps defaulting to "2" templates but only gives me one template name to enter. And when I do enter it (even when I change the # to 1), it won't let me do anything else. The "next" button also disappeared.

Thank you! I ran Find Formmule Errors, and it turned out I had hidden columns with blank headers. I deleted all the columns without a header and now it appears to be working.
