Stuck on setting # of templates and send conditions

to NV Form Mule Add-on

Setting up to use Formmule for the first time on a Google sheet. I can't get past the 2nd screen of the set up. I have chosen to use 1 template and typed in a name, but there's nowhere to select the condition of Null, not null, a number, or anything. There's not even a button at the bottom to go to next or save anything.
