Please Help!! formMule sending emails to all the templates' recipient

I've set up two templates in formMule and the trigger is "send from submit".

After made a change on one of the templates' "To" recipients' email, suddenly it sent email to both templates' email recipients on the  "To" field.

I've tried and delete both templates and re-created them but no luck to fix it.

I also check the tamplates and couldn't find any issues. 

Could someone please help? Thanks so much.

Are you sure you set filters in step 2.
