From the key, the errors that are present say that template sheet name(s) have been deleted.
In Step 2: 'Set # of templates and send conditions', is it possible the names of the email templates were deleted and not re-written?
Essentially Form Mule is trying to match the email template names from the spreadsheet to the email template names in the module and is getting stuck on this step.
You can check if you can open straight up to this step from the Form Mule drop-down.
You may encounter the same spinning wheel though and it might just be easier to reset up this job on a new spreadsheet.
