Unable to add more response template

I had added 3 response template in the start now i want to add 2 more kindly guide the steps

Look at the note in the description of the group.  You may have a problem with your headings.

Normally, you would just change the 3 to a 5.

but once i select 5 from the dropbox there is no addition template tab opening up for me to add the new one.

I even tried creating a new form - there i couldnt even see the template addition button

When I change the number, additional filter options appear.

If I follow the prompts, I see that there are new templates to be edited.
