Formmule menu items missing

I'm running Google Sheets in Chrome Version 142.0.7444.163 (Official Build) (64-bit)

My school has a free educational Google Workspace.

We have formmule running for email notifications for a holiday request form. They stopped working in October. On investigation I can see formmule is still installed but there are no menu items in the Google Sheets, Extensions menu. If you click on FormMule - Email Merge Utility we only see the Help menu item.
