Unable to set up email template

Can you help? I have used FormMule many times but now I am stuck on the setup screen. When I click on 'NEXT: Template and Send Conditions' nothing happens. I made a copy of my sheet and it didn't work on the new sheet either.

Look in the description of this group. There is a tool to help locate the error.
Let us know if that doesn't find the error(s).
