Form not working

is there a glitch in the add-on? as of last week emails stopped automatically sending and when i try to reset the conditions on the add on- the wheel just turns

Are you on the second screen?  There is a note in the description of this group.

If it is in the first step it might be easier to start with a new response spreadsheet from the Google Form.
