Send Conditions

to NV Form Mule Add-on

I noticed that emails have not been going out as usual with the form I have had set up since August. When I went into the template to check on settings, The send conditions had disappeared. When attempting to fix, the send condition options were not avaialble as should be. Any advice on how to fix?

Just adding. I updated the sheet for the merge to come from. When moving on to the send conditions, I set the number of templates to 7, but only one line option showed up.
