Thank you for providing your user key and an image of the console.
The only error I can on the backend connected to your user key is that a time trigger error that says Form Mule can not access the spreadsheet ( this is probably for one of your other Form Mule jobs)
I am have not heard of this issue before and am curious where this sheet is being saved, is this spreadsheet on your drive ( not a team or shared drive), have you tried to open in an incognito window, are any other add-ons being grayed out from the Add-ons menu, do you have an add-blocker installed? ( any of these might be blocking access to the add-on)
