Two docs, one "Turned in", the other not from the same student?

I am seeing a single student have two documents listed, one showing as "Turned in" and the other as "Not turned in"? Their Google Classroom history shows only one time it was turned in.

The student turned in their document days before I ingested them and they made no changes to their submission. I see this fairly often as well with many documents "Not turned in" even when a student has other documents that are turn in.
