Repeated Submissions

I use two different google forms using doc appender with the same settings. One of them transfers just fine into a google doc. However, the other repeats each submission I input 3 times. I'm not sure why it's doing this? I'm having to go in manually to delete the "extra" submissions.

Any advice?

I had this issue and the only solution that worked was to make a copy of the Google Form and start over.
