Remove Doc App

Would like to remove the add-on and start over. 
How do I remove it?

Hello,

To uninstall add-ons from Google Forms:
  1. Open a form.
  2. In the top right, click More.
  3. Click Add-ons.
  4. In the top left, click Menu    Manage Apps.
  5. Next to the add-on, click Options Uninstall.
