Doc appender not working

to NV Doc Appender Add-on

I have been using doc appender for years in a google form. I frequently add new Google Docs to it and then repopulate my list I am appending. Then all of a sudden it stopped working. I have tried everything. Making a new google form. Making new google docs to append to. I have also reinstalled the program twice and no success.
