Timestamp - Incorrect Format

Hello,

I am using Doc Appender in a Google Form.  When I open the Google Doc afterwards, the timestamp displays day/month/year, however, I would like it to be month/day/year.  Spreadsheets and general settings has it correct.  Please Help!  I am a teacher and find this tool very useful except for this annoying glitch. See attached example.

I found that if I use the "separate bulleted lists" or "separate vertical tables" on Doc Appender the timestamp is accurate.  For some reason the timestamp is still inaccurate with the horizontal format.  Check it out and see if this works for you, too.
