Not updating on form submission

I am using Form Publisher to create a document to drop into the source folder for Doc Appender.  Doc Appender states, " Doc-picker choices will populate with Google Doc titles from the folder selected in Step 1, and will auto-refresh hourly, and on each form submit."  The app does update the Google Form after an hour or so, but is not updating on each form submit.  Any thoughts?
