Choices vanishing after form submit

Hello,

My school uses this tool for parent/teacher call logs.  I have a folder with individual call logs (one for each teacher).  Each file name corresponds to an individual teacher name.

When teachers submit the Google Form and then go to submit it again, their name has vanished from the Form.  It requires refreshing the form (or possibly having another teacher submit the form?) in order for the missing name to reappear.

I am experiencing the same issue.  My work google account has recently been moved to Google Education Plus.  Any known issues there?  Doc Appender works fine in my personal Google account.

This is still happing almost a year later.  The choice selected vanishes after form submit, and doesn't reappear until after another user submit the form (or until I manually refresh the DocAppender add-on).  

Any support would be appreciated.
