Not pulling all docs from folders or updating on form completion

DocAppender is not pulling docs from a specific folder.  It was initially not updating, so I tried to refresh the list from the sidebar. Then, it stopped working from that folder all together.

I tried changing the folders, and it will pull with some folders but not with others. I moved all the documents I need pulled in the drop down list to a new folder, and it won't pull it from there either.
