I set up a new form in docAppender today, and it is not appending to the form. Everything is set up correctly with the correct file. It pulled all of the students names over to the form when I set it up, but after filling out the form, the feedback does not appear on the document.

Hello,

Is it possible you were logged into more than one Google account ( multiple accounts open confuses permission settings) when you set up the process?
Try resetting the trigger and see if that gets it going.
While only logged into one google account, open up docAppender, click through until you get to Step 4, then you should see a 'disable' button the bottom of the sidebar.
Click disable> refresh your form>open docAppender and go through the set-up again then try a test submission.

I was not signed into more than one Google account. I have tried disabling the form multiple times, but that hasn't fixed the issue.

Is the folder with the documents on your drive or a team drive? And did you create the folder or did Google Classroom create the folder?

Try moving the folder to the top of your drive, not in any subfolders.
As a test, can you create a new folder on your drive with a test document and see if you can get docappender to work on that? ( Sometimes in the past, older folders or folders not created by the owner can have this permission issue)
Also when you open docAppender can you select 'Get User Key' and paste that key on this thread, I'm not sure if there would be any backend errors associated with this but no harm in checking it out.
