Doc Appender totally stopped working

Doc appender worked perfectly just two weeks ago when I set up some forms for students to use to append to their own docs. Now all forms I created last week and this week have all stopped working (the old ones still work, thank goodness). I've tried recreating new forms and going through all the steps, 2-3 times. I have one form that works for ONE student doc, so I've had to submit the form submission to the same doc and manually copy and paste it into the actual student's document. Is this a problem specific to my account? Please help! 

As an aside, I am able to go through all the proper steps (select folder, link docs to the form, select questions and format, and click enable). None of the information actually shows up on the linked doc that I select!

Thank you for providing your user key and explaining your set-up.

There is not any activity on this key, is it possible you were signed into more than one Google account when you retrieved the key?

Also, just to confirm  - are you working with a folder of documents that are on your Drive (not on a Team or Shared Drive)? 

Can you try moving this folder to the top of your Drive ( not in any subfolders) and then while in a Chrome incognito window (important), try to toggle the trigger by opening Doc Appender, click through until you get to Step 4, then you should see a 'disable' button the bottom of the sidebar.

Click disable> refresh your form>open Doc Appender and go through the set-up again to try a test submission.

I was signed into my work Google account through my browser so it should not be conflicting with any other accounts. 

The folder of documents are in my own Drive, and was generated through Google Classroom (as an assignment post). 

I just tried the steps you indicated here and it still didn't work. I disabled DA in a Chrome Incognito window, refreshed the form, and then went through the set up again. Tried a test submission - nothing =(

Could you try as a test to create a folder yourself with a few docs inside and see Doc Appender will work in that scenario?
I am trying to see if Doc Appender does not work for you at all or if Doc Appender is not working on a folder created from Google Classroom. Thanks!!

Hello, it worked! The test folder and manually created docs all worked properly. But it is not practical because everything I do with my students is assigned through Google Classroom. If you can help me resolve this issue and enable DA to work even with Classroom created folders/docs, that would be great.
