Mapping Error on Template

Good morning,

I have a very simple order form that is mapped to a Google Sheets template. It was working with no issue until Autocrat upgraded itself to the latest version. Now there are 2 fields that just show the Tag I have on the template instead of the actual value.

I have deleted the fields on the template and re-added them. Same result. I have deleted the fields on the Form and re-added them. Same result. I have moved them on the form and re-mapped them. Same result. I can't figure out why this is happening. The only thing that works (for a short time) is if I clear the Form sheet (as I"m still testing I can do that), and start over. However, when I do that, Autocrat reverts back to older version and works for a while until it upgrades itself to latest version. Then those two fields don't work again. I'm at my wits end!

Anyone experience something similar???

I should add one more note - If I preview the documents generated via Autocrat job, it displays the two fields just fine.
