AutoCrat is Reproducing/Re-Merging the Prior Previous Entries

Hello,
I create a google form with the sheets to generate a Doc that has the data tags for my weekly lesson plans. Last I did not have the problem I am having now. This is a new sheet and new Autocrat setup. 

My issue is the autocrat is reproducing/re-merging the information on the line prio everytime it receives a new entry. 

For example:

I complete the google form for week 1 math lesson plan. Document is created.....great

I then complete the google form again for week 1 science. Document is created after it creates another week 1 math document. 

I then complete the google form for week 1 english lesson plan. Document is created after it creates another week 1 science document. 

I stopped complete the google form for the lesson plans because of the duplication. 

I am unable to see the error. I have opened last years sheet and autocrat alongside this years sheet and autocrat and see no difference in information inputted and selected. 

Looking for a resolution befor I just use lasts years Google form. 

Thank you in advance for any suggestions, help or resolution.

I'm going to guess that you deleted some data without deleting the entire row.

Autocrat is confused.

Create a new destination from the Google Form Responses.  Set up Autocrat and it should work.
