Most likely your template is a Google slide. Make sure that the merge tags are not hidden by photo background. They cannot be inside a table.

You cannot copy a slide from a Microsoft presentation and have it work.

Same error for me. I am in Slides. I manually typed all of the tags, but they are not being recognized, and now the program is just spinning and spinning. I uninstalled and reinstalled but no luck there.

I cannot delete or unlink the file because it locks up without letting me address that issue.

Any advice is appreciated.

Make a copy of the spreadsheet or select a new destination for the form.

Create a new template and add merge tags to see if it works. 

One at a time, add any information to the template until the template fails.  

You will now know what is causing the problem.

If you can't find the error, share a copy of the template and a spreadsheet with one dummy record for testing purposes.  I will take a look.
