                   Dear Dr Ferenchick

I am a retired physician (Associate Emeritus, Cleveland Clinic, Cleveland, Ohio). I saw
your discussion with your father online concerning the unusual observation that you have made
concerning the Covid 19 patients. I understand that you have frequently observed that the arterial
oxygen concentration of some of the patients was 85%, but "they looked comfortable". You may or
may not know that patients with the vitamin B1 deficiency disease beriberi were known to have low
arterial oxygen concentrations and high venous concentrations, obviously implying that something was
wrong with cellular pickup of oxygen from the lung and its delivery to cells. Perhaps by measuring the
venous oxygen concentration, you may be able to support this possibility. It also might answer the
question mark hanging over the chest x-ray that you describe as "looking like pulmonary edema". The
finding of increased lactic acid dehydrogenase, elevated CRP and other inflammatory markers are quite
consistent with findings in thiamine deficiency.
I am sure that you must be familiar with the fact that many severely ill patients in the emergency room
are found to be thiamine deficient and thiamine was included in the intravenous treatment of sepsis
patients reported by Marik et al. You can check me out by typing in my name in Amazon books to read
the reviews on our book "Thiamine deficiency disease, dysautonomia and high calorie malnutrition".
The book records many case reports of successful treatment of mystery patients using megadose
thiamine for treatment.
I decided to try to communicate with you in writing. I have no doubt that you would appreciate that this is
an urgent matter if I am correct and I hope that you will give me credence for an idea that seems unlikely.
                                   Yours very sincerely,
Derrick Lonsdale MB BS, FAAP, FACN, CNS
Please note; thiamine blood levels are almost invariably normal
