maternal/OB unit "Clinical" orders

I am looking for any input related to how other sites handle their outpatient clinical patients.  For example, currently when a patient is ordered from the office for a weekly NST, the office calls the Maternity/OB unit, this is then added to the schedule book, and an outpatient clinical account is created for the initial NST, following clinical accounts are created later after being scheduled.  There is currently no order placed into the medical record as it is not written by the physician.  

Do these patients need orders entered as it is completely outpatient?  Should the provider be completing a paper requisition for the test that should be scanned in?
