I chose the k-means algorithm for clustering in wireless sensor network
I have to implement in ns3
 I do not know how I have to initialize the cluster-head in first step

we don't have a clue either. Moreover, why are you thinking that we should have ?

I mean, the algorithm choice and implementation is your, it's not natively provided by ns-3. As a consequence, it's kinda normal that we have no ideas.
