Suggestions on Practice Address fields

• Provide better guidance on what to put in which field for the Practice Address.  Some use Address 1 for the practice/dept name, some put it in the second, some place the suite # in one of the fields, etc.  Address 1 should be the Street Address only, Address 2 can contain the other info.
• Do not allow P.O. boxes in the Practice Address.  Any practice address must have a physical location where services can be accessed.  See NPI# 1396793824
• Ability to list multiple practice locations under an individual’s NPI# - single practice address is often not descriptive of the current locations where a provider provides care.  Providers  should be able/requested to list all practice locations where they spend, say, at least 25% of their time. 
• Also, adding a telemedicine flag for providers that see patients through that modality would be useful as this care is independent of the provider’s actual practice location at that time.
