The server that I originally installed ZFD 4.0 on is down, possibly for
good. Assuming I cannot resurrect it, how do I go about installing v6.5
on a different server?

if the server is down for good, install the new server with the same
information and install zdm 6.5 (better 7) the same way like you did zfd
