I would want to remove the DMZ from the picture until we know the clients
and MT are working properly.
Then troubleshoot DMZ specific issues.

The server was originally on the LAN and worked ok. The Firewall
people say that the ports are open between WAN-DMZ and DMZ-LAN. I can
telnet to port 389 from the LAN, the ZMT Server can ping the main
eDirectory server (so the route is ok).
NSADMIN is giving me an AuthXTier login, I have reinstalled ZMT and
repatched to SP2 (once without changing credentials) and again by
entering in the login/authentication details.
